In Sierra Leone they have "solved" the problem by training Community Health Officers to do the work of doctors. The CHO is a 3-year training program, usually after a nursing qualification. They do all the same work as a doctor, but because the qualification isn't recognised outside of West Africa, there's no "brain drain". However, there's also no chance for them to study abroad to improve their skills. Also, the training is focussed on spotting the most common problems and treating empirically; it's not the same evidence-based training as a medic.
