I don't fully understand your question, but not all rashes need a dermatologist. And dermatologists are expensive. It's actually the opposite of "trying to get more money out of you." Most people present with common dermatologic conditions, and it's more efficient for the PCP to diagnose it rather than overburden the few dermatologists we have on common conditions. The ones that the PCP is uncomfortable diagnosing/treating get referred out.
