My experience with engineering would say that a proper SIL (safety integrity level) analysis had not been done.

Had the MCAS review identified the importance of the AOA sensor during a SIL review they would have mitigated the hazard by using both AOA sensors outputs to increase the unlikelihood of the issue happening because the chances of both sensors failing would have been 1 in millions ... almost impossible.
