What's your most memorable encounter with a member of the audience, be it good, bad or otherwise?

Has there ever been a performance where something has gone really wrong, and the audience never knew?

Holy cow, that's serious. I hope the knight was OK in the end.

Has this happened more than once? Because I remember being at a show where something like this happened. But maybe it was her panties? Either way, wasn't very appropriate for a family-friendly venue. I remember my parents were horrified.
