I think u/Evanderson is referring to the urban legend that this was not in the script and was left in the movie because the "drunk extra" hit you on the first try. I think this answer pretty well debunks that myth.

The scene when a can is thrown at John Malkovich's head is real. An Extra became drunk and decided to pull off the prank. Director, Spike Jonze, loved the scene so much, he put it in the film. Subsequently, the said actor has received a SAG card for this infamous scene.

Man, I was really expecting the stock "I can't choose, they're like my children" answer. I guess I will need to check this one out.
