I'm curious do American planes have to switch to metric measurements (feet to meters, miles to kilometers) when they fly to Europe or anywhere and vise versa for European planes flying to America? Or do the ATC's simply know how to switch back and forth in their head and do so?

Also what about language barriers? Does Heathrow have someone that speaks every language out there to communicate with pilots or do almost all pilots speak English?
