price setting is not exclusive to single payer and every country with universal coverage has price setting regardless of how many payers there are. Maryland also does this in its hospitals so regardless of what insurance you have (or if you have none), you are charged the same amount and that's the amount you/your insurer pays.

pharmaceuticals are not covered under Canada's single payer program. You can buy pharma coverage or pay out of pocket or if you are low enough income then it is provided

why don't more states use all payer rate setting for hospitals like Maryland does?

when we talk about pharma in the US, we talk about outpatient. Why would we be talking about that differently elsewhere
