The fatal dose of most opioids is many times larger than the effective dose, so how many of these overdoses were deliberate? What percentage of the fatal events were from mixing multiple drugs?

I have taken it up with our benefits manager because my health insurance paid out an additional $168 to the doctor (on top of $300 to the doctor for the wellness exam, plus they want $25 from me) because I answered the question that the doctor asked me about my health, during a visit that is supposed to be about checking on my health.
