My wife and I are monthly donors to PIH, we were recently featured in the PIH newsletter which so far is the highlight of my year. I saw that you were doing an AMA and I just wanted to make you laugh. I know you get this all the time, but sincerely thank you for all that you do.

If after the wars in West Africa, which ended over a dozen years ago, there had been substantial investment in health systems strengthening, then Ebola wouldn’t have been able to mow through these countries

The question is, how do we create that investment in health systems? You've emphasized in the past the lack of investment in the government of Haiti, which created an acute-on-chronic situation for the country in 2010 when the earthquake hit. Is it that we need to encourage our governments to offer more in foreign aid specifically for health systems? (A hard sell in the US since the foreign aid budget is a favorite target for some politicians).

I have a question about adding fluoride to water. Here in my city we recently had a scandal involving our mayor who unlawfully ordered our water treatment workers to stop adding fluoride to the water. (It was unlawful because the mayor doesn’t have the power to unilaterally change something that’s in city code). He later claimed he took action because the workers were complaining about how they hated it, that it irritated them even with ppe. No worker complaints regarding fluoride had ever been documented with the city, the union, or OSHA. What has your experience with adding fluoride been, and does that excuse sound plausible (why or why not)?
