The first guy to use doctor assisted suicide in Oregon suffered from ALS. He was the dad of my wife's friend. He wanted to go out on his own terms. Seems like a very reasonable course of action. However, OP stated above that he wants to live as long as possible. Doesn't sound like he's considering suicide.
