There’s also a large difference between those who are addicted to pills and those who take them as prescribed and have a physical dependence on them. Opiates are not the only prescribed medication that causes physical dependence. There’s a lot of medications that do. It is one of the few that unfortunately has a street option. There’s also people in chronic, intractable pain, who can’t afford to see the doctors needed for decent treatment and buying street drugs (not using to get high) and living life is cheaper for them.

The system is broken and people are dying on all sides.
