I have the #3 issue too, so it's definitely not an error on the client.

I'm running Firefox 24 beta releases, have not tried it with the stable releases yet, if this helps.

It is a model that has worked well in other countries so the company I work for, MINES ADVISORY GROUP wanted to try it in Laos and it is an effective method.
