The fact is the fifth dentist does agree, but saying "5 out of 5 dentists" makes the advertising sound fake. It doesn't sell toothpaste as well.
All dentists (to my knowledge) recommend toothpaste with the ADA seal. So, any toothpaste with the ADA seal may advertise as being recommended by 4 out of 5 dentists, which is true, or 5 out of 5 dentists, which is also true.
