What comes first when you have a new idea for a book: the themes, the intention/obstacles/conflict, the characters, or something else entirely?

Follow up: at what point during writing the book does the title become apparent?

I think I can answer this: because along with all the people that are being transported via that bus, there's also a bunch of books, merch, bags (this is a LOOOONG trip), instruments, etc. Transporting all that on a plane would be ridiculously expensive. It's also why many very popular bands also use buses when traveling short(er) distances.
