This is a guess, but lots of practicing surgeons take mini working vacations to do charitable work abroad, but would never give up their very lucrative practice back in the US (in fact, their lucrative practice is what allows them this luxury). This timeline probably allows those surgeons to be involved, and they're a scarce resource.

Non-surgeons are much more numerous, and much more likely to do this as a career choice, so they'd be easier to get for the long haul.

The other thing I can think of is that, by and large, surgery is the same regardless of the country you're in, and you can drop in and do it without much of a learning curve. The medicine itself, though, is potentially very different, even if you just look at infectious diseases alone, so medical types probably have a longer learning curve.

What I meant by surgery not varying is that you can take a trained American surgeon and drop them in anywhere that has basic surgical equipment and they can do their job. The only significant difference would be surgeries that require very specific equipment, like neurosurgery or some cardiac surgeries, or if the surgeon only knows the minimally invasive version of a procedure, but in those cases they just don't do it. All the other things you mention (demographics, HIV prevalence) is all true, but irrelevant because virtually every surgery boils down to something like "remove parathyroid gland" or "cleft palate repair".

I'm curious - why the difference re: kids here? I've been thinking shotgun is safer than a handgun because it's harder for a small child to use, and easier to secure.

I admittedly haven't had a chance to play Numenera yet, but I am trying to get my group together to play it, and looking through the character foci I noticed that the majority of them relied on Intelligence, which would imply that they're of more benefit to Nanos then Glaives or Jacks.

I understand that it's probably easier to create foci that act like esoteries, but any plans in the future to create more Speed- and Might-based foci?
