Locomotive brakes turn on when disconnected (i.e. fail-safe), but the brakes run off an air reservoir which can be depleted if the brakes are on for too long. When the cars are not in use they're supposed to have the manual brakes applied.

Phase I clinical trials test safety, not effectiveness.
Imagine what would happen if you started by testing the vaccine in HIV negative people and discovered that the virus was not completely inactivated.
