How similar/different are mental asylums from mental health departments in hospitals? Like is a mental asylum for the severe cases of mental health patients?

I started volunteering in the ED at a hospital from last year and since then I have worked my way around different departments including behavioural health and palliative care. I will be starting with mental health very soon but I was always curious to know what the most challenging field is in terms of patient interaction. Hence my question, which is now from what I see is probably mental health, so thanks for your reply!
