$0.30/min is extremely low. Have you considered a site like NiteFlirt? You set your own prices and make 70% of what you charge. My phone-only lines are $2.49/min and I make $1.61/min from that.

Edit: your answer to #3 is disconcerting, too. Landlines and AIM are both old technology, and you should have the right to turn down any calls you are uncomfortable with.

I've worked in this industry full-time (camshows, videos, and phone chat) for a few years and have never catered to any kink or fetish I was uncomfortable with. I have also never tolerated clients treating me inappropriately. I've hung up on guys who tried forcing me into something I wasn't okay with, or talked to me in a manner I found disrespectful. At the end of the day, being happy with what I do means more to me than being happy with the money I make doing something I'm not okay with.

It's pretty standard, to be honest. I don't know anyone who is in this line of work who doesn't draw lines and set boundaries. You don't need to sacrifice your sense of self to be successful in this industry.

This only applies to more remote locations. With mobile phones being the more common form of telephone communication these days, it's pretty much become a requirement that they be made more reliable. I've only ever used my cellphone for phone sex, and I've never had it lose or drop a call.
