What do you feel is the largest hurdle to getting out of homelessness? How do we actually help those in our area?

I work for an organization that is trying to help connect people to the social services they need (from problems like homelessness to food insecurity to medical situations). So it's a question constantly on my mind. Especially living somewhere with a large homeless population.
