Hey I work at a major hub in the US.

I'll tell you the things that you don't really get to know from the inside looking out.

But most of the time if a bag doesn't get to your destination when you do it's because it's been dropped out of a cart and it's sitting out there until someone picks it up, figures out what it is, and then sends it on its way.

I drive around the ramp and I see a dropped bag almost every 5 minutes.
