To help you understand the psychological harm - as a 15 year old boy, imagine taking your clothes off in front of a girl for the first time and having blotches of de-pigmented skin on your body, your genitals, etc.

I also suffer from vitiligo and I had a LOT of insecurity when it spread to my genitals when I was in my late teens. It never seemed to actually matter in relationships, but I never had any one night stands or similar flings as I was too embarrassed to have someone see me without a solid relationship.

Bleaching surrounding skin is also a common "treatment" for vitiligo, and I am sure MJ was doing that to minimize the visibility.
