I'm not OP, but form what I've heard the supplemental questionnaire are for borderline students (students who they haven't put into the 'accepted' and 'rejected' groups).
So if you didn't get one, they have decided to accept or reject you already.

I have questions:
- Is it difficult to have a double major at UCLA?
- When are acceptances sent out?
- What is the effect of applying to an impacted major? (like CS)

It's not a country since it is self-proclaimed only. It is unrecognized by other countries and international organizations.
