Who decides who needs medication and what medication they should be on?

I think therapeutic communities are the most similar to this asylum. In TCs the patients and therapists live together but the patients are responsible for their own and other's mental health. They will also work together to ensure the community runs effectively. It aims to build responsibilities and empathy for each other. Has mixed results
