Repeated, rhythmic movements like brushing your teeth (and I imagine jerkin it, though I've never seen the results clinically) can look like perfect V-tach on an EKG...

He has asthma, which he raps about on war and on PTSD, and when we get bad copd/asthma exacerbations in the hospital we usually nebulize albuterol in their lungs and start them on prednasone or solumedrol IV... These last two are steroids and will make you gain weight, sometimes very significantly, and he may have been on them after the hospital too... He probably had to get off of them before any real progress could be made regardless of activity level, and after he would have had to work it all off

Also, while in the throws of an asthma exacerbation, he probably could barely get out of bed, let alone do cardio
