I have a brother in law with some issues. I guess he's one of those people who a decade or two down the line may do something really fucked up and everyone will be like "yea he was always a bit weird". This is about you not him so I won't go into too much detail on his problems. However from an outsider's perspective his biggest issues are twofold:

  1. He doesn't realise how much he hurts the people who love him. My in laws seem to have aged 20 years in the past 10 from dealing with him.

  2. He refuses that there is anything wrong with him and will not seek help.

Don't let this be you. This will not go away on its own. See a doctor.
