The dam had a faulty turbine which failed. Turbine vibrations caused fatigue damage to over 90% of eight bolts in the turbine cross-section (of 49 recovered bolts, 41 had fatigue cracks), which led to failure.

There was no "Accidental cyber event", unless he's talking about a different dam.
