At my location we'd be reprimanded just for touching the meat without gloves before putting it on the grill. Dropping a patty and actually tossing it on there would cause a fucking meltdown.

As someone that also works at McDonalds (Admittedly in a different country, though also in Western Europe): Burgers sit in the heater for a maximum of 20 minutes before they're tossed. Batches are tracked and monitored constantly and we're reprimanded if old food sits in the heater for any amount of time.
