To what extent is anybody actually held responsible for such deaths? Do they continue because there's so little responsibility?

I recall reading that antibiotics might be capable of permanently damaging gut flora. Is there any credence to this notion? As a researcher, what's your take on the ubiquitous use of antibiotics in general?
