Something simple I'll do with older patients is miscalculate their ages. I'll have an old lady tell me her birthday and let's just say she was born in 1937. I'll ask and say "that makes you 29 years old, right?" Or they'll tell me they're 78 and I respond with "nooo you mean 32!"
Usually gets them to laugh or smile, and that immediately helps calm them down and make them a little at ease.

EMT here, I ran a call back in august that had a similar effect. Bad car crash and the guy was conscious, but combative as hell and had to be sedated.
The best way I heard it described is as if the lights are on at the house, but nobody is home.
He's awake, but doesn't know it and won't ever remember it.

For us it's usually the days leading up to and immediately after the full moon that shit really hits the fan on both the fire and ems side.
