What's the process of writing an article like? Most importantly, how do you pick a topic to write on?

I have a feeling he meant cookie (biscuit in UK).

Any possible reasons given by medical professionals you've spoken to? Diabetes (feel like this would be sort of obvious though), vagus nerve damage, demyelinating disease...?
