How often do people who get wishes recover from their conditions? Do people ever hold back from applying for wishes in case they might recover and 'waste' the wish?

I suspect the /pol/ effect may be at least partly in play. It starts out with everyone being completely ironic. But some people start being genuine, with latent and repressed desires coming to the forefront, and/or genuine people coming in from outside. It's possible for things to get bad enough that the actually ironic people leave.
