What single social change and single policy change when implemented at a global level would have the greatest impact?

Thanks Jos,

I think that's neatly side-stepped my question :-) but on reflection you've given the only answer you could do ;-)

My follow-up question is buried below but is:

Does the environmental impact of increased populations and the pressure they exert on fragile ecosystems (over subsequent generations) form part of the decision making process when embarking on an immunisation program?

Essentially is there a trade off between solving a health and humanitarian problem in the short-term for an environmental and social problem in the medium term (due to increased population pressure over the next generations)? If so... are there mitigating solutions during the implementation phase?

For example, I assume immunisation programs are associated with female education programs so that family size can be reduced thereby reducing ecosystem impact (albeit with a lag over a number of generations). This view is based on the research (or at least Ted talk) of Hans Rosling.
