It's common in many companies to set performance targets such that you have to either ignore safety guidelines, or else fail to meet the target. I'm not saying that's what happened here, but I wouldn't be surprised.

It can also happen organically, if performance targets get set based on actual performance. The people setting the targets may not be aware of what is happening.

In either case, it is then easy to blame the worker -- but in neither case does any individual worker have much choice. They'd have to organise to stop such practices, and unions are an endangered species in USA.
